If the whole story is ever published, it's gonna turn out to be operator error. Too coincidental to be anything else. I'm glad everybody survived, but they all paid for the operator's stupidity.
They didn't close everything up before they started pumping, they didn't sniff test the bilge after fueling, they didn't run the blower for a few minutes before starting the motors (if they even shut them down during fueling- I've seen that idiocy, too.). Or some combination of the above.
Not enough people know proper fueling procedure, and only some of those actually follow it every time.
