I have a great running 1968 fastwin 18hp, i obtained a parts motor which is a 1969 fastwin 18. i always thought my cam follower was odd on my 68, when i was tearing down the 67 model i noticed the cam follower is of a standard design but mine is not , does anyone have any info on why or which is better?

I think you are asking about the cam, not the follower. I'd hate to say which is "better". They probably are not interchangeable if that is what you are thinking. To the best of my knowledge, the newer one is supposed to give better fuel economy at mid-range throttle.

yes cam, not follower. also it is interchangeable, only the curvature of the cam the follower rides on is different. I'm assuming the larger one will provide better mid range.
