1995 50 hp merc rectifier burnt up

what would cause the rectifier to burn up. mywarning horn was going off every now and then. I took of the elec cover and found my rectifier was toast

reversed batt. cables,loose batt. terminals,disconnecting batt. cable when eng. is running ,just to mention a few
