1995 Johnson 70 hp burnt piston troubleshooting

I am waiting for my block to come back from boring and honing so I can start rebuilding it. The previous owner burnt a hole in the center piston. I was warned to figure out why the motor burnt the hole since it will just happen again so I am trying to figure out how to troubleshoot it. What have a noticed so far?

1 - the tell tale was very weak when the engine was running and when I took apart the water pump it was not sealed and was missing all the o-rings
2 - The ignition module on the center cylinder was replaced, but that should not affect a lean condition to lead to exhaust piston failure

What do I plan on doing?

1 - soak the carbs in a carb cleaner dip tank that I bought at autozone
2 - replace the carb needle and seat (recommended by the repair shop)
3 - inspect and replace fuel and oil hoses as needed

What else should I clean/replace when putting the motor back together?

How do I determine if the center cylinder is getting oil/fuel?

Should I put the carbs back in the same order to swap them around?
