The impeller rotates clockwise looking at the front of the housing, and before launching the boat go over everything. Do you lights work, what about the blower, and make sure your bilge pumps work, since it has been sitting for 4 years i would change out the u joint bellows for piece of mind. Once the impeller has been changed out run the engine for 15 to 20 minutes in your driveway checking temps making sure they are good and not over heating. As for the nipple, if it is where i think it is, it is a block drain for the coolant. Oh make sure to check timing and dwell on the motor aswell

Makes no difference in the way you install the impeller.
It will orient itself as soon as the motor turns over.
The nipple might be a drain for the block? Pics?
Running in the drive:make sure the water pickup is sealed.
There is a 1/2 in hole on the very bottom of the drive that should be plugged for running in the driveway.
