98 Mariner 225 EFI

I've been running it on the muffs a few times to diagnose another issue and there's a thick black oily sludge coming thru the prop/exhaust.
So far, I've changed plugs, and plug wires, had the injectors cleaned and serviced, verified fuel pressure and verifed spark to all 6 cyls.
Smokes quite a bit too.

How concerned should I be about the sludge?
I want to take it out and run it but she's still running a little rough and I don't want to risk heading out on the water and causing damage if there's something else i should be checking.

Sounds like the motor is getting too much oil. Have you verified how much it's taking per gallon of gas? Probably need to correct the oil ratio problem and run a can of Power Tune through it to clean it out.
