If I just slap in a long block from a truck (4.3 Chevy) with the car cam and heads will performance suffer?

It's basically the same, but marine engines have brass freeze out plugs, & stainless head gaskets. If you're boating in fresh water, then it's not as critical to change them, saltwater you must. You must also use all the marine external component, including the starter, for safety reasons (no explosions) & Coast Guard regs, Otherwise, you should be fine!

If you're going to do that, why not leave the truck spec cam in the block? It's closer to the marine cam than the car one is. There have been quite a few threads about using truck blocks here - search them up...

As mentioned, Core plugs, cam(and if it is a truck or van engine, just use the cam that isin it) and head gasket are different for a marine engine. Hit it with some paint, add the marine externals form your existing engine and hit the lake.
