My owner's manual says I should decarbonize my 2004 4stroke Johnson (115HP) but I'm not sure how to do it, or how to tell if it is truly needed. I don't live near a marine dealer so will have to drive an hour to get a can of whatever is needed. My local autoparts store carries Seafoam though.

you should not have any substantial carbon build up. My suggestion however, is since SeaFoam is available in your area, use it in the fuel as a preventive measure. Do a search for "Decarb" on this forum. It is covered in a number of threads.

The techron/techroline additive in chevron gas is about as good as it gets for cleaning a 4-stroke. Run a few tanks of that, or go get a bottle of it from the auto parts store and add it in. Seafoam is a good choice too. Whatever you choose, you probably don't need to go the full route of 1-can:1-gal for a decarb like a 2-stroke.
