Have you replaced the impeller in that outboard? If not, that would be the next step. Most of the water comes out near the prop. The holes are just an exhaust bypass, so some water comes out of there, too.
A tell-tale is nice, and my 92 6hp has one. I like to be able to glance back to see that water is circulating.
In the meantime, while your outboard is on the boat and you're on the water, try pulling the cowling off and running around, using the engine normally. Once in a while, reach over and put your hand on the cylinder. You should be able to leave it there for a few seconds before it gets too hot.
Also, if you're running the outboard in a barrel, you should have the water level well up the leg. I like to put mine in so the water is halfway up the leg. If it's too shallow, you won't get good circulation.
You don't actually need a tell-tale. If you have a good impeller and the water passages are clear and the thermostat is working, you'll be fine. Just check the temperature from time to time with that hand test.
At idle, some water should splatter out of the relief holes. At higher throttle settings, there will be more. It's not a precision thing, but you do need to have the leg of the outboard deep enough.
