If it is on 3 blocks(two rear corners and bow), you slide the trailer under it until the cross member hits the bow block. Now jack up the bow, in front of the cross member and reblock it. Now slide the trailer under until the next cross member hits, and repeat the process. Safety is utmost. Take your time and make sure the jack and blocks are on flat ground and are stable. When it is all the way on the trailer, drop the tongue and remove the rear blocks.

AND be sure to attach the winch strap and keep taking up the slack.

Only thing I'd add to above is that if the blocks the boat is on hold it more than 4-6" above the trailer bunks, be very careful to make sure of each move. If it falls more than a couple of inches at any time, it can cause a lot of damage. It would really help us to make helpful suggestions if you could paste a picture of the boat on the blocks.
