Need help locating the right PTT, please help!!!

I have a 115hp Johnson, year is 1975, here are the model and serial numbers.

Model- 115ESL75E
Serial- J4322208

Any help would be appreciated, thank you in advance. I have been trying to find the right one for a couple weeks now and can't match up the numbers to have one that fits without making changes.

I'm guessing you meant by PPT Power Trim and Tilt?
