OMC 140 Question

I have a 1975 OMC 140 and I cannot get it to start right. In order to get it started I have to turn the carburetor all the way as lean as it will go and it will start on the first turn, but as soon as it goes I have to throttle it all the way up and the boat barely does 5-10 mph. I have to go to engine while the boat is running and put it really rich and it runs perfect besides the fact I have something wrong with one of the pistons and it clicks. If I try to start it while i have it running rich, it will just not start at all. It won't turn over or anything, the starter just turns the crank over and over and over. As soon as I put it back to lean it starts up again. Is there another adjustment I have to change? Is there something wrong with my carburetor? Please help!
