Yes, they do work.
You can see if your outdrive starts leaking oil, the level drops in the bottle, not in the drive, and you don't end up with dry gears in the upper.
If your drive oil gets water in it, the gear lube in the reservoir will turn start turning milky.

I know the ones that came with them stock (alpha gen 2 and bravo) work well but my concern is this add-on kit which screws into the vent hole. The line seems too small to be effective. Do you fill it the same way, just fill the bottle from the bottom plug too, to make sure all the air is out?

You fill the drive like normal. I just fill the reservoir, let some drain out the hose to displace the air in the hose, and connect it to the drive.
