problem on cold startup

i have a 88 johnson 88 spl.. for some reason starts fine on the hose put if try to start cold in a lake ..will not start.. i believe only difference is the resistance on the exhaust in the water.. no problems once warm

Yes, the backpressure is the difference. There's a few things it could be but lets start at the beginning / basics -
