Volvo 4.3 SX Serpentine Belt Issue

I have a 2008 195FX with a 190 HP volvo engine with only 14 hours. The last two times I have been out after about an 1 1/2 hours the belt has fallin off. The belt is new and shows no signs of wear nor does it appear to be loose. Any ideas what is causing this.

check alternator has all the bolts in it and that one hasn't come out and therefore twisting it .this sort of thing is enough to pop the belt.if all looks ok 1/4"-1/2"max movement.
