Volvo 5.7Gi Starting issue

My 2006 Volvo 5.7Gi always needs a small amount of throttle on startup, whether warm or cold. If I turn the key when hot, she just won't catch and needs a small amount of throttle. Once started she idles immediatly and runs great. If I rev her, she returns to below idle briefly and almost dies, but recovers and then idles great.
Seems to be using more fuel than normal and will not operate above 3900rpm, even at WOT.
Is it the fuel pressure as the fuel pump whines quite loud, or idle control/throttle bypass sv.
Fuel filter and plugs have been changed, air filter/flame arrestor is clear.

Any help would be apreciated.

Sounds like it's too rich on start up. Might want to get it hooked up to a diagnotic tool and find out what is going on. Could be the ECT lying to the ECM with how cold the engine really is.
