Wagner Velvet Drive...Drive Shaft Spun Off!?

Working on a Mercruiser 350 with a Borg-Wagner Velvet Drive # 1017004. It's a friends boat, he just bought it, 2nd day out it won't go into gear....just growls. Gets towed back and I enter the picture. Upon arrival I notice that the prop is actually touch the steering plate or rudder (not sure what the term is) which is obviously a problem. It didn't appear that the shaft was damaged so I just pushed it back up and threaded the shaft back into the v-drive.

Why did it come out in the first place? It seems like there is a keyway on the driveshaft, is that what came out? There is of course the 4 hex screws on the c-clamp to help support the shaft but that couldn't possibly be the only thing holding that drive shaft in right? Anybody happen to have the part # or parts catalog so I can help them order a new key for the keyway assuming that's what it is?

Also, the trans fluid is now milky (I assume with the drive shaft uncorked they were probably sinking and allowing water into the v-drive) Do I need to do anything special to the drive to be certain no long term damage or just drain and refill? While you kind sirs are reading already, I'm unfamiliar with these motors and don't know where the impeller is to change it for them. The intake seems to run up along port to a small box under the port side seat and back. Is the impeller in there?
