Anyone have any idea what would have caused this hole to be put into the side of this lower unit (there's one on the opposite side too) on a Johnson V4?

It's surface corrosion (and clearly due for a scrap and paint). The material around the hole is pushed outward on both sides so I suspect something inside did it, but I can't think of anything in that area that would have broke through the casing like that.

I have a theory,
the neighbor got drunked up and took a pot shot at your boaty. You should start inviting him out for a boat ride now and then and he won't be as jealous of you.

you can try to clean it up some...scrap it down ..lay a piece of widow screen over then and apply some JB weld..or have it welded ..but if it rotten the weld may cause more damage...try the JB Weld..that stuff is strong..get the marine version...I think NAPA or Autozone in Southbridge have it....maybe even Wal Mart in Oxford

This happened over a year ago when it was running, but I suspect there may be some damage to the lower unit. The boat was pulled and the unit drained (only water came out) and then I sprayed it with fogging oil to keep it from corroding. But do you think it's worth rebuilding and having it welded?

is there really any moving parts up there beside the drive shaft..water pump is above it...looks close to the oil resovoir...does the gears turn ok....is there oil in them now...cant think of any moving parts in that area

I agree with Al. In fact when I saw that pic it put me in mind of some foreign object holding moisture (or something more corrosive) against that aluminum long enough to eat a hole through it. Almost like it was submerged, with a piece of steel or something butted up against it.
