Has anyone used a company to make a new plexiglas windshield using their old windshield as a template? Who did you use, are you happy with the result?

look locally for plastic fabrication and sales. most companies that sell acrylic sheet also do heat forming, etc. many will advertise boat windshields, etc if they make windshields
agree with Matt, many of the local glass shops that do Auto/Residential/Commercial will have a company or two that they do business with if they do not do plastic fabrication in-house.
