Does Mercruiser sell a kit for this or do I have to order parts individually?
Yes OEM Merc # 8M0095485,
http://www.ebay.com/itm/New-OEM-MerC...-/171533782721
Or aftermarket Sierra # 18-8219,

Also, some diagrams show the tapered insert on both sides of the hose and some only show on the side on the exterior of the transom. Is it on both sides and is it the same part? The part of the hose on the interior of the transom is so fubar I can't tell what parts I see there.
Depending on what kit, kit part number, time frame the kit was packaged, what individual intake water hose part number/design you receive, will determine if you'll get the tapered inserts at both ends of the hose or not. The later designed OEM hoses are now coming from Merc with a plastic tube/pipe that gets inserted into the gimbal housing hole where you found the blockage. The new style hose hopefully can be seen in the photo below, (< If not, it's in one of the links posted).

MarkSee might be able to add a note to this as he just recently installed this new style hose and did his first test run today and all went well on the cooling system. Looks to me this might be an answer from Merc to cure some of the bravoitis problems. ^^^ Oops, I didn't see Marks post until after posting this.
So, it looks like I just need the gasket 27-441081, the connector 22-43828, the hose 32-43437, 2 inserts 41674T, and the insert tool 91-43579. I do not need that brass fitting, right? Part number 33 on the link below 22-428611

And nope, you won't need the fitting plug # 33.
