Engine mount question.

I'm going to be reinstalling what used to be a 1982 merc 228 back into my boat.

I noticed the two back motor mounts are not at the same elevation in the engine bellhousing. There is 3/16 difference in height.

If I can get the engine to align properly with the gimbal bearing should I be concerned?

Yes, one mount has collapsed, you need to replace both to save the coupler

Agree. Knock both mounts out and install 2 new ones. The old mounts have been superseded. The new part number is 8M0103028. Follow the new direction carefully.
