Fire Out Of Emtey Hole?

I have a freind who just bought a boat with a 40hsp Mariner-
He said when he has the top plug pulled he sees fire spit out the top cylinder when cranking ?
He pulls the bottom with the top in no fire comes out the top
Just starting out on this one & looking for help

could be a hole in the cylinder wall between 2 cylinders?? i dont know just a stab in the air? doesnt sound good tho. the cylinders should be seperated with a plug out, it must be getting ignition somewhere??
