Having just done my own all you need is two automotive relays 5 pin 12 volt 25 - 30 amp and two push switches, the relays need to be single pole change over relays . I have uploaded an master piece for you to study here...

connect + and - of the battery to the same normally open and closed contacts of the relay , make sure you put an inline fuse in the posi leg feeding the circuit i think 20amp should do , the motor goes across the common of each relay , when either of the relays is energised it will make the circuit and will reverse the motor depending on which relay is pulled in . The supply to relay coils i just fed via some push switches and have one for up and one for down. works a treat
For three wires I think one wire is negative with seperate up and down cores, in this case just supply 12volt via the same relays to each core
