It depends on what motor we're talking about. If it's an outboard then you'd be running on the reverse gear which isn't intended to take the load the forward gear is. Again, depending on what motor it is.

Some outdrives are intended to be run either way, some aren't. Inboard tranmissions are the same. Some can run either way, some aren't designed that way.

It's a Mercruiser Sterndrive Alpha 1 5.7. I have been having some issues with wierd cavitations and no acceleration. I'm running a 21 pitch right now and my WOT is 48 MPH @ 4200 RPM. Engine range is supposed to be 4400-4800 RPM. I was thinking about going down to 19 pitch to try and get a better hole shot.

Yes, just what you think, it would go backwards, while in forward, & vice versa. Yes, from the info given, I'd say a 19" prop should do the trick, I don't know what material prop you've got now, but you probably could even use a stainless prop also.
