Going down to base metal is always best but not necessary. If you're going with the same color then you don't have to worry about it chipping and seeing another color underneath and can just sand down the rough or rusty spots so you don't see the transition from paint to bare metal. I would be careful about grinding because you'll likely see the grinding marks thru the new paint. Sandblasting is the best way to get down to base metal.
