The correct answer is: It depends!
Replacement pistons are balanced to factory weight, even in oversizes.
If the cylinder bores are in good condition, you need only to replace one piston. This can be done without disassembling the engine.
If the cylinder bore(s) are scored, then the affected cylinders must be overbored and oversized pistons installed. This requires disassembling the engine. If only one or two cylinders are affected, only they need to be done.
If three cylinders on a four cylinder engine are affected, you can do only three but you might as well do all four and in essence have a brand-new engine.
So: It comes down to the condition of the engine, amount of money you can afford, and how you feel about replacing pistons. GIMME MY HUNDRED! LOL
