Hi G.M.<br /> Generally, exhaust comes out of two places on most modern outboards. The holes above the waterline are "exhaust relief ports", which are needed to help with low speed and neutral idling. If all the exhaust were to exit under water in neutral it would effectively drown the motor, as it wouldn't be able to overcome the backpressure of trying to push the exhaust out underwater. Once in gear, most of the exhaust will exit underwater, as forward motion and propeller movement will draw the exhaust out...<br /><br />- Scott
