Hello All,

I would like to know if water is suppose to come out of the hole, on this 89 Johnson 48 spec O/B (or) should it only be exhaust fumes.

That hole is the exhaust relief. Yes, water will come out there. Water will exit there, typically, only after the thermostat has opened, but not always.

Yes you should get water out of there with the exhaust
the cooling water from the block drops in the exhaust housing (mid section)
