3 years ago, was just the impeller replaced or also the cup and plate?

The blades can take a set from sitting and not being used which means they will break easier. Using it more often is better than leaving it sit. But it's your decision.

I agree with Boobie here. It is irrelevant if it was used little, as he stated that is worse. They do wear from use, but worse, they break from sitting. Rubber dry rots right.

Ever had to pull chunks of rubber out from within a powerhead? Not fun. My 2 cents is to just do it.
