There are several other points that can have an icing "waived off," in the NHL game that is. If the team shooting the puck down the ice was attempting a pass (that could have been played by their teammate) or if the defending goaltender moves from his crease to attempt to play the puck the linesman can call for play to continue. This is by and large a judgment call and not frequently called anymore.
As for the chaos factor of hockey. It does often look chaotic, especially on television, but that is primarily due to the speed of the game. The game is played in a very reactive way, especially on the defensive side, which often makes it seem like the players are unsure of where to be. The better players, Crosby, Ovechkin, Lidstrom, Pronger, etc. tend to be proactive making their opponents move into undesirable and sometimes uncomfortable positions.
Watch some footage of the Russian Five from the Detroit Red Wings or the 1970's Montreal Canadiens and you can start to see just some of the plays that are actually in place with many of today's teams.
