I bought a boat last fall with a broken engine. I got a rebuilt 4.3 and got it installed in the winter. Also, I performed an oil change on the sterndrive. I took it on the water this last weekend. After a while on the water, I noticed that the oil from the sterndrive was spilling over from the oil reservoir cap. I'm wondering if the oil and water are mixing. The oil looked light blue(ish). Perhaps similar to, but with a yellow tint like a milkshake when engine oil is mixed with water.

If it's mixing oil and water, what could be the issue?

be that the seals in the outdrive are leaking and oil and water mixes. I think also the vent on cap can cause pressure build up and overflowing as well.

Would crack your drain screw on outdrive and see if you have milk there
