That box on the inlet hose may be a remote oil cooler, not a filter. You probably have an air leak in the lower unit set-up somehow. I would pull the lower unit and see if the o-ring seal on the transom shield to the lower unit upper gearhousing is intact or looks aged-smashed-damaged. Replace it either way. Pulling the lower unit is not as difficult as it seems. It is made easier with an engine hoist, but I have done it with blocks of wood and my back. I'm going through this same deal, not proper cooling in the lake, but cools great on my garden hose attachment tube in the engine bay (mines a 2004GL-E, so no muffs required) while in the driveway.

Re: volvo penta 5.0 gl overheating

I know my volvo does not like to pump water off of the muffs. I dunked it in the river, on a week day evening, when the launch ramp wasnt busy. Fired it up, pulled the outlet hose off the pump. and almost filled the bilge with the amount of water it was pumping! Reconnected, and never went over 130deg. My original overheat problem was a thermostat corroded shut, and a water outlet on the drive that was corroded where the hose connected. Mine is older than yours, so I am not sure if your out drive has a water outlet. If it does, I would check it for corrosion, plus I used aviation sealer at all the hose connections to make sure the hoses sealed. Considering your raw water pump is on the crank, I assume you do have a water outlet on the drive. Good luck, and keep us posted!
